Care consultation links the family and/or the affected person to
resources specific to the individual’s and family’s needs. These
resources may include: memory evaluation services, adult day
programs, support groups, home care programs, personal care and
homemaker services, counseling and referrals to legal and financial
professionals. The care consultant talks with the family to:
- Assess needs and available support systems
- Assist with problem-solving and developing a care and service
plan
- Provide and review educational materials
- Suggest strategies for improving safety in the home
- Offer tips on handling difficult behavior, improving
communication and understanding the disease process
- Provide referrals to community resources
- Provide guidance and advocacy throughout the application
process for various government-funded programs.
- Provide regular follow up and assistance as needs change
This program is particularly useful when the caregiver is having
difficulty determining appropriate service options, is discouraged
by trying to navigate the health care system, is a long-distance
caregiver, or is overwhelmed by caregiving responsibilities. The
Care Consultation Program is staffed by a licensed master of social
work (LMSW) and has been available since 1996. Since that time, the
program has served over 800 families.
The program is available statewide. Home visits are available to
families in Androscoggin, Cumberland, Kennebec, Sagadahoc, and York
counties; visits outside these areas are scheduled occasionally.
Office visits, telephone consultation, and email consultation are
available to families statewide. There is no fee for the service,
but contributions or membership in the
Alzheimer's Association, Maine Chapter are appreciated.
